The Mississauga-Halton LHIN project has the following primary objectives:

  1. Describe the populations of supportive housing, CCAC, LTC homes, and CCC hospitals in the MH LHIN using available interRAI data, wherever possible. When such data are not available, it will be necessary to complete new interRAI assessments to provide the information needed for the project.
  2. Examine the appropriate service environments for the sampled populations based on staff assessments. For persons with a completed interRAI assessment, staff will be asked a series of questions regarding their current and future suitability for different service environments.
  3. Pilot test the applicability of the interRAI Community Health Assessment (CHA) and its supplementary modules for use in supportive housing.
  4. Demonstrate the utility of the interRAI suite of instruments to support MH LHIN decision making regarding resource allocation and service planning related to the continuum of care.
